ABSTRACT

Objective: This cross-sectional investigation aimed to determine the incidence, clinical characteristics, prognosis, and related risk factors of olfactory and gustatory dysfunctions related to infection with the SARS-CoV-2 Omicron strain in mainland China. Methods: Data of patients with SARS-CoV-2 from December 28, 2022, to February 21, 2023, were collected through online and offline questionnaires from 45 tertiary hospitals and one center for disease control and prevention in mainland China. The questionnaire included demographic information, previous health history, smoking and alcohol drinking, SARS-CoV-2 vaccination, olfactory and gustatory function before and after infection, other symptoms after infection, as well as the duration and improvement of olfactory and gustatory dysfunction. The self-reported olfactory and gustatory functions of patients were evaluated using the Olfactory VAS scale and Gustatory VAS scale. Results: A total of 35 566 valid questionnaires were obtained, revealing a high incidence of olfactory and taste dysfunctions related to infection with the SARS-CoV-2 Omicron strain (67.75%). Females(χ2=367.013, P<0.001) and young people(χ2=120.210, P<0.001) were more likely to develop these dysfunctions. Gender(OR=1.564, 95%CI: 1.487-1.645), SARS-CoV-2 vaccination status (OR=1.334, 95%CI: 1.164-1.530), oral health status (OR=0.881, 95%CI: 0.839-0.926), smoking history (OR=1.152, 95%CI=1.080-1.229), and drinking history (OR=0.854, 95%CI: 0.785-0.928) were correlated with the occurrence of olfactory and taste dysfunctions related to SARS-CoV-2(above P<0.001). 44.62% (4 391/9 840) of the patients who had not recovered their sense of smell and taste also suffered from nasal congestion, runny nose, and 32.62% (3 210/9 840) suffered from dry mouth and sore throat. The improvement of olfactory and taste functions was correlated with the persistence of accompanying symptoms(χ2=10.873, P=0.001). The average score of olfactory and taste VAS scale was 8.41 and 8.51 respectively before SARS-CoV-2 infection, but decreased to3.69 and 4.29 respectively after SARS-CoV-2 infection, and recovered to 5.83and 6.55 respectively at the time of the survey. The median duration of olfactory and gustatory dysfunctions was 15 days and 12 days, respectively, with 0.5% (121/24 096) of patients experiencing these dysfunctions for more than 28 days. The overall self-reported improvement rate of smell and taste dysfunctions was 59.16% (14 256/24 096). Gender(OR=0.893, 95%CI: 0.839-0.951), SARS-CoV-2 vaccination status (OR=1.334, 95%CI: 1.164-1.530), history of head and facial trauma(OR=1.180, 95%CI: 1.036-1.344, P=0.013), nose (OR=1.104, 95%CI: 1.042-1.171, P=0.001) and oral (OR=1.162, 95%CI: 1.096-1.233) health status, smoking history(OR=0.765, 95%CI: 0.709-0.825), and the persistence of accompanying symptoms (OR=0.359, 95%CI: 0.332-0.388) were correlated with the recovery of olfactory and taste dysfunctions related to SARS-CoV-2 (above P<0.001 except for the indicated values). Conclusion: The incidence of olfactory and taste dysfunctions related to infection with the SARS-CoV-2 Omicron strain is high in mainland China, with females and young people more likely to develop these dysfunctions. Active and effective intervention measures may be required for cases that persist for a long time. The recovery of olfactory and taste functions is influenced by several factors, including gender, SARS-CoV-2 vaccination status, history of head and facial trauma, nasal and oral health status, smoking history, and persistence of accompanying symptoms.

ABSTRACT

Myocardial infarction (MI) is one of the leading causes of death in the world. With the improvement of clinical therapy, the mortality of acute MI has been significantly reduced. However, as for the long-term impact of MI on cardiac remodeling and cardiac function, there is no effective prevention and treatment measures. Erythropoietin (EPO), a glycoprotein cytokine essential to hematopoiesis, has anti-apoptotic and pro-angiogenetic effects. Studies have shown that EPO plays a protective role in cardiomyocytes in cardiovascular diseases, such as cardiac ischemia injury and heart failure. EPO has been demonstrated to protect ischemic myocardium and improve MI repair by promoting the activation of cardiac progenitor cells (CPCs). This study aimed to investigate whether EPO can promote MI repair by enhancing the activity of stem cell antigen 1 positive stem cells (Sca-1+ SCs). Darbepoetin alpha (a long-acting EPO analog, EPOanlg) was injected into the border zone of MI in adult mice. Infarct size, cardiac remodeling and performance, cardiomyocyte apoptosis and microvessel density were measured. Lin- Sca-1+ SCs were isolated from neonatal and adult mouse hearts by magnetic sorting technology, and were used to identify the colony forming ability and the effect of EPO, respectively. The results showed that, compared to MI alone, EPOanlg reduced the infarct percentage, cardiomyocyte apoptosis ratio and left ventricular (LV) chamber dilatation, improved cardiac performance, and increased the numbers of coronary microvessels in vivo. In vitro, EPO increased the proliferation, migration and clone formation of Lin- Sca-1+ SCs likely via the EPO receptor and downstream STAT-5/p38 MAPK signaling pathways. These results suggest that EPO participates in the repair process of MI by activating Sca-1+ SCs.

ABSTRACT

ObjectiveTo investigate the prognostic value of the enhancement pattern in arterial phase of preoperative Gd-EOB-DTPA enhanced magnetic resonance imaging (MRI) in evaluating the disease-free survival (DFS) and overall survival (OS) in patients undergoing curative resection for intrahepatic cholangiocarcinoma (ICC). MethodsA retrospective analysis was done on the clinical, preoperative MRI findings and postoperative follow-up results of 93 pathologically confirmed ICC patients undergoing surgery in our hospital between January 2018 and December 2021. Kaplan-Meier survival curves and log-rank test were used to compare the DFS and OS of three groups with different arterial enhancement patterns. Cox regression analysis was used to identify the factors affecting DFS and OS. ResultsThere were significant differences in DFS and OS among the 3 groups (log-rank test, P < 0.05). The arterial enhancement pattern was an independent predictive factor for DFS (using diffuse hyperenhancement as a reference, peripheral rim enhancement: HR = 3.550; 95%CI: 1.16 ~ 10.8; P = 0.026;diffuse hypoenhancement: HR = 3.430; 95%CI: 1.04 ~ 11.3; P = 0.042). The arterial enhancement pattern and tumor location were predictive factors for OS ((using diffuse hyperenhancement as a reference, diffuse hypoenhancement, HR = 8.500; 95%CI: 1.09-66.3; P = 0.041; using tumor distal location as a reference, tumor perihilar location HR=2.583,95%CI: 1.14-5.83, P =0.022). The AUC of arterial enhancement patterns in predicting 1-, 2-, and 3- year DFS were 0.722, 0.748, and 0.617, respectively; in OS, 0.720, 0.704, and 0.730, respectively, which showed better prognostic efficacy than AJCC-TNM staging system. ConclusionArterial-phase enhancement pattern of preoperative Gd-EOB-DTPA enhanced MRI is an independent predictive factor for DFS and OS of ICC patients, with a better prognostic value than AJCC-TNM staging system, and can be used for the clinical management of ICC patients.

ABSTRACT

Objective: To understand the characteristics and trend of the premature death rate of 4 major chronic diseases in Ji'nan from 2015 to 2020. Methods: The death cause surveillance data and population data during 2015-2020 in Ji'nan were collected, and abbreviated life table, Joinpoint regression analysis and other methods were used to analyze the characteristics and change trends of the premature death rates of 4 major chronic diseases. Results: The crude mortality rate and age standardized mortality rate changes for the 4 major chronic diseases from 2015 to 2020 range from 568.65/100 000 to 604.06/100 000 and 366.77/100 000 to 432.48/100 000, respectively. The annual premature death rate of 4 major chronic diseases declined by 3.33% averagely from 2015 to 2020 (95%CI: -6.25%--0.32%), which might be explained by the declines of the premature death rates of cardiovascular and cerebrovascular diseases [average annual percentage change (AAPC)=-3.23%, 95%CI: -6.32%--0.05%] and cancer (AAPC=-3.58%,95%CI:-6.83%--0.21%). The average decline rate in women (AAPC=-4.19%,95%CI:-7.56%- -0.70%) was higher than that in men (AAPC=-2.92%,95%CI: -5.65%--0.11%). Conclusions: The premature death rate of 4 major chronic diseases showed a downward trend in Ji'nan from 2015 to 2020. Men should be considered as a key population in the prevention and control of 4 major chronic diseases, and attention should also be paid to the non-significant declines in the premature death rates of chronic respiratory diseases and diabetes.

ABSTRACT

Traditional Chinese medicine (TCM), which owns abundant chemical components and complex action pathways, has been widely recognized in the prevention and treatment of diseases. Some analysis methods have been emerged in order to ensure the quality of TCM and to develop new TCM drugs. Matrix-assisted laser desorption ionization mass spectrometry (MALDI-MS) is a soft ionization mass spectrometric technique with the advantages of high throughput, high sensitivity, low cost and so on. It provides technical support for the molecular level study on TCM. At present, this technique has been used in the field of composition analysis and metabonomics research of TCM, and plays an important role in the identification of Chinese herbal medicines, real-time molecular screening and the construction of metabolic network pathway of active ingredients. Among them, the selection of appropriate matrix and sample preparation technology is the key to ensure the detection effect of MALDI-MS. With the development and optimization of new matrix, the continuous improvement of sample preparation technology and the combination of MALDI-MS with various analytical methods will greatly improve the detection effect. Based on this, this paper discusses the application of MALDI-MS in TCM, including high-throughput detection of active ingredients in TCM, monitoring of the original medicines and their metabolites in vivo, and in situ visualization and characterization of tissue distribution information of active ingredients in TCM. It also discusses the application prospect and existing problems of MALDI-MS in TCM, so as to provide technical support for the identification of active ingredients in TCM, drug utilization and metabolism.

ABSTRACT

The paper introduces professor GAO Shu-zhong's understanding on "seeking yin from yang needling method" and its clinical application on the basis of "qi street" and "four seas" theories. Through professor GAO's clinical practice for years, he integrates and extendes the theories of "seeking yin from yang", "qi street" and "four seas" in Huangdi Neijing (The Yellow Emperor's Inner Classic). In this specific acupuncture method, in reference with the theories of "qi street" and "four seas", acupuncture is exerted on yang part of body, e.g. the back and lumber region to treat the diseases of yin parts, e.g. the chest and abdomen, which is differentiated as yin-yang imbalance in pathogenesis. In order to fully explain the clinical curative effect of "seeking yin from yang needling method", the common diseases in clinic, e.g. the disorders of heart, spleen and stomach systems, as well as the gynecology are taken as examples in the paper.

ABSTRACT

OBJECTIVE To establish a method for simultaneous determination of zeaxanthin ,β-carotene,β-cryptoxanthin palmitate and zeaxanthin dipalmitate in Lycium barbarum . METHODS L. barbarum was extracted with n-hexane-anhydrous ethanol-acetone-toluene(10∶6∶7∶7,V/V/V/V)by ultrasonic method. High performance liquid chromatography (HPLC)method was adopted. The determination was performed on YMC C 30 column with mobile phase A consisted of methanol-acetonitrile-water (81∶ 14 ∶ 5,V/V/V)and mobile phase B consisted of dichloromethane (gradient elution )at the flow rate of 1.0 mL/min. The column temperature was set at 20 ℃. The detection wavelength was set at 450 nm,and sample size was 20 μL. Using zeaxanthin as control, the relative correction factors (RCFs)of β-carotene,β-cryptoxanthin palmitate and zeaxanthin dipalmitate were calculated , and then the content of each component was calculated according to RCFs and compared with the results of external standard method(ESM). RESULTS The linear range of zeaxanthin ,β-carotene,β-cryptoxanthin palmitate and zeaxanthin dipalmitate were 0.119 4-2.983 8,0.121 7-1.521 6,0.285 9-5.718 8,8.460 5-211.513 3 μg/mL(all R2>0.999). RSDs of precision ,repeatability and stability(16 h)tests were all less than 4%. The average recoveries were 103.34%,107.37%,105.64%,96.16%(RSD<5%,n= 9). The average RCFs of β-carotene,β-cryptoxanthin palmitate and zeaxanthin dipalmitate were 1.109,1.390,1.158. The relative errors of the content determination results by quantitative analysis of multi-components by singer marker (QAMS)and ESM were within ±1%. CONCLUSIONS The established HPLC-QAMS method is accurate and stable ,which can be used for the content determination and quality control of 4 carotenoids in L. barbarum .

ABSTRACT

In this study, a method was established for in-situ visualization of metabolite distribution in the rhizome of Paris polyphylla var. yunnanensis. To be specific, through matrix-assisted laser desorption/ionization-mass spectrometry imaging(MALDI-MSI), the spatial locations of steroidal saponins, amino acids, organic acids, phytosterols, phytoecdysones, nucleosides, and esters in rhizome of the medicinal plant were directly analyzed, and six unknown compounds with differential distribution in rhizome tissues were identified. The specific procedure is as follows: preparation of rhizome tissue section, matrix screening and optimization, and MALDI-MSI analysis. The results showed that the steroidal saponins were mainly distributed in the central, amino acids in epidermis and cortex, low-molecular-weight organic acids in central epidermis, phytosterols in the epidermis and lateral cortex, the phytoecdysones in epidermis and cortex, nucleosides(uneven distribution) in epidermis and cortex, growth hormones around the epidermis and cortex, particularly outside the cortex, and esters in cortex with unobvious difference among different tissues. In this study, the spatial distribution of meta-bolites in the rhizome of P. polyphylla var. yunnanensis was characterized for the first time. The result can serve as a reference for identifying and extracting endogenous metabolites of P. polyphylla var. yunnanensis, exploring the synthesis and metabolism mechanisms of the metabolites, and evaluating the quality of medicinal materials.

ABSTRACT

The paper introduces professor GAO Shu-zhong's experience in qiguan (umbilicus pass) theory and its clinical application. Professor GAO believes that the umbilicus is the "pass" where the primary qi of sanjiao transported from the lower jiao to the middle jiao. It is the general pivot of qi transformation of yin and yang, as well as the place for qi ascending, descending, exiting and entering in the human body. Hence, the umbilicus is called qiguan (umbilicus pass). In clinical practice, associated with observation, palpation and pulse diagnosis, the qiguan theory is conductive to disease diagnosis. Moreover, the therapeutic methods for promoting qiguan is generated, i.e. umbilicus-acupuncture therapy and umbilicus-moxibustion therapy. In the umbilicus-acupuncture therapy, Gao 's umbilicus five points (umbilicus heart, umbilicus stomach, umbilicus liver, umbilicus kidney and umbilicus lung) are commonly selected. With the umbilicus- moxibustion therapy, the isolated moxibustion with different herbal materials is exerted at the umbilicus, in which, the herbal materials with drastic medical action, pungent and fragrant in flavor and warm in property are specially selected.

ABSTRACT

Whether direct manipulation of Parkinson’s disease (PD) risk genes in the adult monkey brain can elicit a Parkinsonian phenotype remains an unsolved issue. Here, we used an adeno-associated virus serotype 9 (AAV9)-delivered CRISPR/Cas9 system to directly co-edit PINK1 and DJ-1 genes in the substantia nigras (SNs) of two monkey groups: an old group and a middle-aged group. After the operation, the old group exhibited all the classic PD symptoms, including bradykinesia, tremor, and postural instability, accompanied by key pathological hallmarks of PD, such as severe nigral dopaminergic neuron loss (>64%) and evident α-synuclein pathology in the gene-edited SN. In contrast, the phenotype of their middle-aged counterparts, which also showed clear PD symptoms and pathological hallmarks, were less severe. In addition to the higher final total PD scores and more severe pathological changes, the old group were also more susceptible to gene editing by showing a faster process of PD progression. These results suggested that both genetic and aging factors played important roles in the development of PD in the monkeys. Taken together, this system can effectively develop a large number of genetically-edited PD monkeys in a short time (6–10 months), and thus provides a practical transgenic monkey model for future PD studies.

ABSTRACT

Traditional Chinese medicine (TCM) is the treasure of the Chinese nation. As an important raw material for clinical treatment of diseases, Chinese materia medica plays an extremely important role. However, in the process of transformation from traditional wild collection of animals and plants to modern artificial cultivation and industrial production of preparations, whether the quality of Chinese materia medica is fully transferred will directly affect the quality and clinical efficacy of Chinese materia medica preparation. From the field to the sickbed, process control of quality transfer of Chinese materia medica is the key to guarantee quality and curative effect. In this paper, the whole process that affects the quality of Chinese materia medica preparations such as seed and seedling, planting and breeding, harvesting and processing, processing of decoction pieces and preparation production was analyzed. Paying attention to the whole process of quality control of Chinese materia medica is of great significance to improve the quality of Chinese materia medica preparations and promote the rapid development of TCM. Based on this, the author intended to analyze the key control links in the quality transfer process of Chinese materia medica (breeding, planting areas and field management, timely harvesting and intensive primary processing, appropriate processing, optimization of preparation technology, standardization of packaging and informationization of storage and transportation), in order to provide reference for the design and development of Chinese materia medica preparations guided by clinical value.

ABSTRACT

Objective To explore the characteristics of plantar pressure of ankle joints at different limb laterality and different stress positions. Methods Twenty-three healthy subjects were recruited to perform walking trails. Each subject was tested for plantar pressure in ankle neutral position, ankle inversion and ankle eversion through simple custom-designed walkways and plantar pressure plate. The evaluation indices were peak pressure, contact area, contact time percentage, M/L (ratio of sum of medial plantar peak pressure to sum of lateral peak pressure), A/P (ratio of sum of toe peak pressure to heel peak pressure). Results The peak pressure of the first metatarsus bone in the dominant foot was significantly larger than that in the non-dominant foot, and the peak pressure of the fifth metatarsus bone was significantly smaller than that on the non-dominant foot. The M/L of dominant side was significantly larger than that of the non-dominant side. Except for the peak pressures of the mid-foot and the first toe, there were significant differences in other regions among the three ankle positions. The percentage of contact time in each area during inversion was greater than that in neutral position, and the percentage of contact time of plantar regions during eversion was longer than that in neutral position except the second toe. The M/L in inversion, neutral and eversion position were 1.24±0.46, 1.06±0.26, 0.88±0.25. The M/L of dominant foot was greater than that of the non-dominant foot, and the A/P during inversion and eversion was greater than that in neutral positions. Conclusions The stability of the dominant foot was better than that of the non-dominant foot. The standing stability decreased during inversion and eversion. During inversion, the body shifted inward and forward to maintain the stability. During eversion, the center of gravity shifted outward and forward to maintain the stability.

ABSTRACT

Objective::To establish a pre-column derivatization reverse-phase high-performance liquid chromatography (RP-HPLC) method for the simultaneous determination of 17 amino acids in Cynomorii Herba from different producing areas and conduct a multivariate statistical analysis. Method::RP-HPLC with pre-column derivatization was employed, with phenyl isothiocyanate (PITC) as derivatization reagent. Separation was performed on a WondaSil C18-WR column (4.6 mm×150 mm, 5 μm), with 0.05 mol·L-1 sodium acetate solution (pH 6.5) as mobile phase A, and acetonitrile-methanol-water (3∶1∶1) as mobile phase B for gradient elution at a flow rate of 0.8 mL·min-1. The detective wave length was set at 254 nm, and the column temperature was maintained at 35 ℃. Principal component analysis (PCA) and systematic cluster analysis (HCA) models were established for multivariate statistical analysis and quality evaluation. Result::17 Kinds of amino acid were detected in Cynomorii Herba, 7 of which were essential amino acids. The 17 amino acids showed good linearity in respective concentration range, r = 0.999 0-0.999 9.The average recoveries were between 98.03%-103.89%with RSD<3.5%. The results of PCA and HCA were basically the same, and both methods can be used to clearly distinguish Cynomorii Herba from 12 municipal producing areas into 6 regions. PCA can be used to classify Cynomorii Herba according to different municipal or provincial production areas, and HCA can be used to classify it according to provincial production areas. It showed that the amino acid contents in Cynomorii Herba from different municipal and provincial producing areas had differences, and the content distribution showed obvious geographical clustering characteristics. ABSTRACT

@#AIM: To observe the changes of anterior chamber angle after posterior chamber intraocular lens(ICL)V4c implantation for high myopia.<p>METHODS: Prospective study. ICL V4c implantation was performed on 150 cases(300 eyes)with high myopia, and the changes of visual acuity, intraocular pressure, vault and anterior chamber angle were recorded before and 1d, 1mo, 3mo, 6mo and 1a after operation. Repeated measurement analysis of variance and LSD-<i>t</i> test were used to analyze the data.<p>RESULTS: After 1d, 1mo, 3mo, 6mo, 1a operation, the recovery of UCVA was significantly higher than that before operation(<i>F</i>=98.2, <i>P</i><0.01); there were statistically significant differences in AOD500, AOD750, TISA500, TISA750 and SSA of nasal side before and after operation(<i>F</i>=108.256, 112.342, 164.856, 316.549, 115.338, <i>P</i><0.01); there were statistically significant differences in AOD500, AOD750, TISA500, TISA750 and SSA of temporal side before and after operation(<i>F</i>=102.68, 548.47, 93.37, 316.549, 117.698, all <i>P</i><0.01); the parameters of each angle decreased gradually in the 1d, 1mo and 3mo after operation on both sides, and there was statistical difference in the two comparisons(all <i>P</i><0.05), but there was no statistical significance in the parameters of each angle in the 3mo, 6mo and 1a after operation(all <i>P</i>>0.05).<p>CONCLUSION: After ICL V4c implantation, the anterior chamber angle parameters in the horizontal direction of 1d, 1mo, 3mo, 6mo and 1a were smaller than those before operation, and tended to be stable after 3mo.

ABSTRACT

OBJECTIVE: To study the chemical constituents from Melilotus officinalis(L.) and their anti-oxidant activities. METHODS: DPPH Assay and β-carotene bleaching assay were used for screening the fraction of anti-oxidant activity in the 70% ethanol extract of Melilotus officinalis(L.). The isolation and purification were performed by silica,Sephadex LH-20 and preparative columns. The structures of the obtained compounds were identified by physicochemical properties and spectral data. Then their anti-oxidant activities were evaluated. RESULTS: The ethyl acetate fraction from Melilotus officinalis(L.) showed the most significant anti-oxidant activity. Thirteen compounds were isolated and identified as kaempferol(1), luteolin(2), quercetin(3), hesperidin(4), rutin(5), apigenin(6), salicylic acid(7), coumarin(8), scoparone(9), 7-methoxycoumarin(10), fumaric acid(11), caffeic acid(12), and p-hydroxybenzoate-4-O-α-D-mannopyranosyl-(1→3)-α-L-rhamnoside(13).Compounds 2, 4, 7 and 12 exhibited strong scavenging activities against DPPH free radical with the IC50 values of 25.89, 14.09, 28.03, and 23.04 μg•mL-1, respectively; compounds 4, 7, 12 and 13 exhibited strong scavenging activities against ABTS free radical with the IC50 values of 10.48, 23.77, 22.79, and 25.20 μg•mL-1, respectively. CONCLUSION: Compounds 3, 7, 9, 10, 11, 12, and 13 are isolated from this plant for the first time. Compound 13 is a new compound not reported in the literature; compounds 2, 4, 7, and 12 have strong ability to scavenge DPPH free radicals; compounds 4, 7, 12, and 13 have strong ability to scavenge ABTS free radicals.

ABSTRACT

Objective: To observe the anti-inflammatory and anti-mycoplasma effects and safety of Xiyanping Injection on mycoplasma pneumonia in children, and to explore the mechanism of treatment of mycoplasma pneumonia, so as to provide a new basis for clinical treatment. Methods: A total of 72 children with mycoplasma pneumonia were randomly divided into treatment group and control group, 36 cases in treatment group and 36 cases in control group. The control group was treated with Azithromycin. The treatment group was treated with Xiyanping intravenous drip on the basis of the control group and observed for 7 to 11 d. The therapeutic effects of TCM syndromes, the disappearance time of fever and cough were compared between the two groups to detect serum levels of inflammatory factors. Results: After 7 to 11 d of treatment, the symptoms of the two groups were improved, and the marked efficiency of the treatment group was significantly better than that of the control group (P 0.05). There was no significant difference in serum IL-10, IL-2, IL-4, and IFN-γ between the two groups after treatment (P > 0.05). Conclusion: Xiyanping Injection combined with Azithromycin can significantly improve the clinical symptoms of mycoplasma pneumonia in children with wind-heat stagnation of lung syndrome and phlegm-heat obstruction of lung syndrome, and improve the curative effect of traditional Chinese medicine syndromes, with better safety and no obvious adverse reactions.
